Employee Accommodations

The North Orange County Community College is committed to complying with all laws that protect qualified individuals with disabilities. Therefore, when requested, the District will provide a reasonable accommodation for any known physical or mental disability of a qualified individual, provided the requested accommodation does not create an undue hardship for the District or pose a direct threat to the health or safety of others in the workplace or the requesting employee.

The District complies with all federal and state laws concerning the employment of persons with disabilities, including the Americans with Disabilities Act (ADA) and the California Fair Employment and Housing Act (FEHA). How to Submit a Request

Workplace reasonable accommodations may be provided to employees who experience functional work limitations, when supported by a physician statement. Please follow the instructions below to be considered for workplace accommodations.

  1. Your treating physician must complete the Physician's Statement Form.
    1. Download the Physician's Statement Form.
    2. If your provider uses their own documentation, please ensure it clearly outlines any functional restrictions or limitations related to your ability to work.
    3. Please note that the District does not require any personal health or diagnosis details. Please do not provide personal health or medical diagnoses in documents submitted to the District.
  2. After completing the Physician's Statement Form or equivalent medical documentation, submit the Employee Reasonable Accommodation Request Form.
    1. Complete and submit the following online form: Employee Reasonable Accommodation Request Form.
    2. Upload your medical documentation and describe any limitation or accommodations you are seeking.
    3. Do not begin this form if you have not completed Step 1.
